Ensure you read and prepare the following before visiting the shop.
Required Items for Your Visit
For Switching from Another Carrier (MNP)

Individuals under 18 must provide a written consent and signature from a legal representative (e.g., a parent or guardian). Click here for more details.
Driver’s licence, My Number card, etc.
*Foreign customers require different identification.
For those with different contract holders and users
Power of attorney
If you wish to process an MNP transfer at a shop, please download the power of attorney form in advance, fill in all required fields, and bring it to the shop within one month of the signing date.
Only family members with matching surnames or addresses can be accepted. Please also bring identification documents of the delegator (contract holder) (copies are acceptable).
Guidelines for Minors
Minors' Attendance
Applications from individuals under 18 must be accompanied by a parent or guardian. Both the minor and the guardian must visit the shop together.
Required Identification
Valid identification is required for both the minor and the parent/guardian.
*Please note that student IDs are not accepted.
Filtering Service
"Anshin Control" contract is mandatory for minors under 18 to use Rakuten Mobile use (550 yen/mo.)*1

Book your visit in advance to ensure priority service, even during busy times.
This also allows you to verify required documents beforehand for a smoother experience.
