Child's identity verification documents

For your child's application, preparing the following documents will ensure a smooth application process:

• My Number Card
• Health Insurance Card and Residence Certificate
• Passport and Residence Certificate

Health Insurance Card or Passport requires a Residence Certificate as supplementary document. One document alone is insufficient for identity verification, so please ensure you also have the supplementary document.
Please note that student IDs are not accepted.

Legal Representative’s Consent Form/Named Payer’s Consent Form

For contracts by those under 18, a consent form from the legal representative (parent/guardian) is required. You can download it from below.

For those switching from carriers not supporting MNP One-Stop, you need to obtain an MNP reservation number from your current carrier before applying. If the names differ, please complete the name change before issuing the MNP reservation number.

Only for those where the contract holder and user differ

For those with different contract holders and users with other carriers wishing to proceed with MNP transfer in-store, you can apply to Rakuten Mobile using a power of attorney.

Please check the cases where you can use a power of attorney before visiting the store.

Cases where you can use a power of attorney

Case 1: Other carrier's contract holder and user visit together

Only available for family members with matching surnames or addresses. Please also bring the delegator's (contract holder's) identification document (copy acceptable).

Case 2: Only the other carrier's user visits

Download the power of attorney form in advance, fill in all required fields, and visit the store within one month from the signature date. Only available for family members with matching surnames or addresses. Please also bring the delegator's (contract holder's) identification document (copy acceptable).

For minors visiting a shop

Please visit accompanied by a legal representative (parent), bringing all the above documents, including those for the guardian.

*For number portability (MNP), please also bring the completed power of attorney form.

• You can apply either through the web application screen or "my Rakuten Mobile" app. For details on "my Rakuten Mobile" app, click here.

• For those under 18 using Rakuten Mobile, an "Anshin Control by i-Filter" contract is mandatory (330 yen/mo.)

*If the legal representative determines that "Anshin Control by i-Filter" is unnecessary, you can cancel the contract by mailing a "Filtering Service Unnecessary Declaration Form".
*If Rakuten Mobile receives the unnecessary declaration form by the 10th of the month following the line contract month, the monthly usage fee (330 yen/mo.) will not be charged.
*The cancellation will be effective at the end of the month after receipt of the unnecessary declaration form.

• When applying at a Rakuten Mobile shop, the child and guardian (parent) must visit together, and identity verification is required for both.

*Please bring the completed power of attorney form, delegator's (contract holder's) identity verification document (copy acceptable), the completed "Legal Reprsentative’s Consent Form" and "Named Payer’s Consent Form". The minor's identity verification document is also required.

• For those under 18 using Rakuten Mobile, an "Anshin Control by i-Filter" contract is mandatory (330 yen/mo.)

*If the legal representative determines that "Anshin Control by i-Filter" is unnecessary, you can cancel the contract by mailing a "Filtering Service Unnecessary Declaration Form".

After application, once the contract holder's identity verification is complete, we will send a completion email to the registered email address.

1. 1. Receive the device

   

   After identity verification is complete, we will deliver the ordered device and SIM card to your specified address.

2. 2. Initial setup

   

   Once you receive the device, please perform the initial setup. You can start using it after setup.

   Check the device’s initial setup method

   • We deliver the product with "Anshin Control by i-Filter" pre-installed for online applications.
   • At the time of delivery, an initial management password for "Anshin Control by i-Filter" is preset. After receiving the device, the guardian (parent) must change the initial management password.

3. 3. Use Rakuten Link

   

   Please complete the registration and SMS verification.

• Take home your Rakuten Mobile device and enjoy the "Rakuten SAIKYO Plan".
• The initial setup for "Anshin Control by i-Filter" will be done in-store when the device is received.
• For customers who don't need "Anshin Control by i-Filter", please mail the "Filtering Service Unnecessary Declaration Form" as instructed in-store.
