• *When obtaining a certificate of residence, please prepare one that does not include your Individual Number (my number).
• *If your permanent address and Individual Number (my number) are included, please take a picture and upload it with only the part of your permanent address and Individual Number hidden by paper or other means.

Certificate of evacuation shelter

• *Please confirm that the evacuation site addresses the reacquired residence.

Request for masking before uploading

In accordance with the October 2020 revision of the Health Insurance Act, please mask the "symbol," "number," "insurer number," and "QR code (if any)" information (e.g., cover it with a sticky note before taking a picture of the document, or use an image editing application or software to cover it up after taking a picture) before uploading the document.
